EyePlan Plus Terms and Conditions

Please read these terms and conditions carefully before subscribing to our Eyecare Plan. By signing up for the plan, you agree to be bound by these terms and conditions.

 

1. Subscription and Payment:

1.1 By subscribing to our Eyecare Plan, you agree to pay a monthly fee of £15 for a minimum term of 12 months. Payment will be collected automatically on or around the 1st of each month.

1.2 The direct debit mandate will be active 5 working days after set up; therefore the first payment date may vary.

1.3 Payment will be taken via Go Cardless can be made via a valid bank account.  You are responsible for ensuring that your payment details are up to date and accurate.

1.4 We reserve the right to modify the monthly fee by providing you with at least 30 days' notice in advance.

 

2. Eligibility:

2.1 You must be registered as a patient with us to be eligible for the plan.

2.2 You must have had an eye examination within the last 6 months before your sign up date.

 

3. Coverage:

3.1 The Eyecare Plan covers a comprehensive range of eye care services, including:

    a) Regular enhanced eye examinations as recommended by your optician, subject to our schedule of appointments.

    b) Emergency eye care services.

    c) Diagnostic appointments to regularly monitor dry eye and ocular conditions.

    d) Discounts on prescription spectacles, dry eye treatments and products; and other accessories.

    e) Discounted or complimentary contact lens fittings and aftercare appointments, as determined by the optician.

 

4. Appointment Scheduling:

4.1 As a member of our Eyecare Plan, you will have access to periodic eye examinations.

4.2 It is your responsibility to schedule and attend the recommended eye examinations. Failure to attend appointments may result in a delay in accessing eye care services or a forfeiture of benefits.

4.3 Appointment availability is subject to the optician's schedule. We will make reasonable efforts to accommodate your preferred appointment time but cannot guarantee specific dates or times.

 

5. Cancellation and Termination:

5.1 You have the right to cancel your Eyecare Plan at any time by providing written notice to your participating optician.

5.2 If you cancel within 14 days of signing up, you will be entitled to a full refund of any membership fees already paid, provided no services have been rendered or benefits utilized.

5.3 After the initial 14-day cancellation period, the plan has a minimum term of 12 months, if cancelled within 12 months all discounts and services will need to be paid off in full.

5.4 We may terminate or suspend your Eyecare Plan in the event of non-payment, breach of these terms and conditions, or if the participating optician ceases to offer the plan.

 

6. Limitation of Liability:

6.1 We will not be liable for any delay or failure in performing our obligations under these terms and conditions resulting from events or circumstances beyond our reasonable control.

 

7. Amendments:

7.1 We reserve the right to amend these terms and conditions at any time. Any amendments will be communicated to you through written notice or via email.

7.2 If you continue to subscribe to the Eyecare Plan after the amendments have been made, it will be deemed that you have accepted the revised terms and conditions.

 

8. Governing Law and Jurisdiction:

8.1 These terms and conditions shall be governed by and construed in accordance with the laws of England and Wales.

8.2 Any disputes arising out of or in connection with these terms and conditions will be subject to the exclusive jurisdiction of the courts of England and Wales.

 

By subscribing to our Eyecare Plan, you acknowledge that you have read, understood, and agree to comply with these terms and conditions.
